Keratoconus literally means ‘conical or cone-shaped cornea’. The cornea is the clear tissue that covers the front of the eye, and is its major focussing surface. It is normally round but with keratoconus, it becomes stretched and thin near its centre, and slowly transforms to a cone shape. The distorted cornea affects the focusing ability of the eye impairing the vision, due to associated irregular astigmatism and myopia. Keratoconus patients develop progressive corneal bulging/protrusion due to an weakness of the corneal collagen. Keratoconus usually begins in the late teens to early 30s and affects men and women. It is significant in 1:2000 of people, but mild cases occur up to ten times more often. One or both eyes may be affected. It may begin in one eye and later involve the other. KERATOCONUS SYMPTOMS The main symptom is blurred vision The main symptom is blurred vision even with glasses. Patients usually seek corrective lenses at first for driving or reading. Keratoconus at this stage is difficult to differentiate from other more common vision defects. The glasses prescription increases with time and usually shows high levels of myopia (short sightedness) and astigmatism. (distortion of vision) Some patients have glare driving at night or from sunshine. If the distortion of the cornea worsens, so will vision. This happens in some patients but in many the condition stabilises. It is not possible to predict whether keratoconus will be progressive or at what rate it will worsen but in general the younger the age and the more severe the disorder when it begins, the more rapidly it is likely to deteriorate. KERATOCONUS TREATMENT IN CAIRO - COLLAGEN CROSS LINKING A transformation in keratoconus treatment that stops it from getting worse Collagen cross linking is an important new treatment that stops keratoconus from getting worse. Cross linking increases the strength of the collagen in keratoconic corneas by 3–4 times. It sis carried out with some of the best equipment available today. - LENS IMPLANTATION (PHAKIC IOLS) An effective treatment after cornea shape is normalised. These are lenses put over the normally present lenses. They work together with the eye’s natural lens which is not removed. In a normal cornea it is capable of correcting even the most severe short sight and astigmatism. In keratoconus it is most effective after collagen cross linking, laser or rings implantation have been done to stabilise the cornea and normalise its shape as much as possible - RGP CONTACT LENSES, ROSE-K LENSES Contact lenses do not change the keratoconus, but cover the protrusion on the cornea so neutralising it. Normal vision is restored to all except advanced cases. Lifelong wear is necessary. - RINGS IMPLANTATION INSIDE THE CORNEA It is to reduce excessive curvature in advanced keratoconus. These are small pieces of plastic inserted into a channel created in the cornea to stretch it and bring it back to near its original configuration. They can’t be felt and require no maintenance. They are normally invisible except on close inspection when they look like a contact lens on the eye. - CORNEAL TRANSPLANTATION A long-established treatment for the most severe cases The center of the patient’s own cornea is completely removed and replaced by another corneal transplant. The transplant is sutured in place of the conical cornea. This is the longest-established treatment for severe keratoconus. - LASER Laser treatment can be used to treat keratoconus in some rare occcasions by using a cornea topographer to guide the laser, or what is called topography-guided treatment. This is an important development, but Dr Ahmad Khalil reserves it to very rare cases, where the cornea is stable, with a minimal refractive error untreatable by other means. • GLAUCOMA TREATMENT What is Glaucoma? Glaucoma is one of the leading causes of blindness in the world. It is a disease that typically affects older people, but it can occur at any age, even in the newborn. Loss of vision is preventable if the disease is detected early and treatment is started. Glaucoma is typically known as a disease of high pressure inside the eye causing damage to its structures, but some eyes are sensitive even to lower pressures-normal tension glaucoma. Types of Glaucoma THERE ARE TWO MAIN SUB-GROUPS OF PRIMARY GLAUCOMA: 1- Open Angle Glaucoma Despite the angle of the eye being wide and open, the tiny microscopic drainage channels outside the angle are blocked. This leads to what we call open angle glaucoma, which is a slowly progressive condition, and because it is a slow process, it can often be missed, This is what we call “the silent thief of sight” 2 - Angle Closure Glaucoma (also known as closed angle glaucoma) occurs more rapidly when the flow of fluid inside the eye cannot pass through the narrow angle in the front of the eye, where drainage usually occurs, causing a rapid rise in pressure inside the eye. THERE ARE OTHER 2 ENTITIES WITH SOME RESEMBLANCE TO GLAUCOMA Ocular hypertension occurs when the pressure of the eye is repeatedly measured as being high, and yet the eye displays no signs of glaucoma damage. Normal tension glaucoma (normal pressure glaucoma) is the opposite, where the nerve at the back of the eye shows signs of glaucoma damage and yet the pressure is always recorded as being within normal limits. THERE ARE SOME EYE DISEASES WHICH CAN RESULT IN SECONDARY GLAUCOMA Among these are eye inflammation, the growth of new vessels inside the eye, which may occur in connection with diabetes or after vascular occlusions at the back of the eye. CONGENITAL GLAUCOMA In some rather rare instances, glaucoma can occur in young infants. This is called congenital glaucoma and can be inherited. This type of glaucoma can be devastating if not diagnosed and operated early enough. Treatment of Glaucoma To control its effects on the eye and its vision is possible, with various options that should be tailored each to individual case. Glaucoma is broad entity with many intricacies inside its diagnosis and suitable lines of treatment. Dr Khalil, after full evaluation of your case will decide what is best for your particular glaucoma situation. often, just the use of one eye drop once a day might be all what is needed for your treatment! What is the best treatment of glaucoma; medicines, laser or surgery? In recent years, the tools available for the treatment of glaucoma, has undergone a phenomenal expansion. This is thankfully due to to the development of new medicines, and new surgical techniques and devices. All has contributed immensely to the fight against this impeccable disease. Depending on the type of glaucoma, its best suitable treatment can be either medicines, with the use of various forms of eye drops or different kinds of surgery. The treatment of glaucoma using anti-glaucoma eye drops: In many cases, glaucoma can be controlled using eye drops that reduce the pressure inside the eye. Dr Khalil will recommend the most suitable regimen for you based on your condition, which can range from only one kind of eye drops once a day to several kinds of eye drops taken simultaneously. Glaucoma management with eye drops requires your commitment, because they must be taken regularly and continuously to be effective. Laser can be used for the treatment of glaucoma in some situations – laser treatment of glaucoma in Cairo, Egypt, Dr Ahmad Khalil Eye Clinic laser – treatment of glaucoma in Cairo (iridotomy), Egypt, Dr Ahmad Khalil Eye Clinic. Certain types and stages of glaucoma respond very well to simple laser treatment, and it can be all what is needed for some cases. Glaucoma surgery can be the best option for the treatment of glaucoma in some cases If the use of eye drops is not good enough to lower the eye pressure as required, and also in particular types oof glaucoma even with no trial of eye drops , glaucoma surgery is used to create a new channel to allow fluid to drain out of the eye. Glaucoma surgery is usually very effective, and you may even find that you need no further glaucoma medications after surgery. The Ongoing Treatment of Glaucoma: It’s important to remember that glaucoma, once diagnosed, will, in a sense, be there for the rest of one’s life. While there are many effective treatments available, you must take your treatment as advised, and have regular check-ups to make sure that you eye pressure is maintained within safe levels for the health of your eyesight. Glaucoma surgery, if thought useful will reduce to a large extent the need for closer follow up visits. • LASER TREATMENT OF THE RETINA RETINAL LASER The laser is an instrument that produces a pure, high-intensity beam of light energy. The laser light can be precisely focused onto the retina (the light sensing tissue at the back of the eye), selectively treating the desired area while leaving the surrounding tissues untouched. The absorbed energy creates a microscopic spot to destroy lesions or weld tissues together. Retinal laser treatment is a painless outpatient procedure, lasting about 10-30 minutes. It may require more than one session to complete the treatment. In most of the cases, the aim of laser treatment is to try to prevent further loss of vision. It cannot regain the vision if already lost due to retinal disease. After the laser treatment, the patient may require to undergo repeat Fluorescein Angiography at a later date to monitor the response, and may require more laser if indicated. DIABETIC RETINOPATHY Diabetes milletus can damage small vessels of the retina, causing them to leak fluid and blood. These changes decrease vision if the central part is affected. Laser is used to seal these leaks. However laser is done to prevent or retard further loss of vision and not to improve vision. In severe changes when new blood vessels have grown (Proliferative Diabetic Retinopathy), multiple sessions of laser are needed to produce the desired effect. This is called Pan Retinal Photocoagulation and is highly effective in preventing severe visual loss. RETINAL TEARS AND HOLES In a retinal tear or a hole without a retinal detachment, laser is done prophylactically to seal the hole and prevent or limit the detachment. AGE RELATED MACULAR DEGENERATION In patients with wet type of AMD not involving the central part, laser can be done to close the Choroidal Neovascular Membrane. This can prevent severe visual loss. However, in patients with central involvement, other specific techniques involving injections of special materials in the eye to prevent further proliferation, and stop existing ones. RETINAL VEIN OCCLUSIONS In venous occlusions, the central retina may swell up due to fluid collection leading to reduced vision. Such cases require laser to decrease the swelling, which may help in improving the vision. Venous occlusions may also lead to new vessel formation that might cause glaucoma (9eye pressure rise), laser causes these vessels to regress. • Keratoplasty Keratoplasty, Corneal Grafting Corneal transplantation or keratoplasty is a surgery to replace an opaque or diseased cornea with a best clarity corneal tissue, which is obtained from a donor. A number of conditions can be treated with keratoplasty at Dr Khalil Eye Clinic in Cairo, Egypt, including: the conical cornea or keratoconus, Cornea scarring, caused by infection or injury, Clouding of the cornea, corneal ulcers, including those caused by infection, or due to complications caused by previous eye surgery. Technique of Keratoplasty Dr Khalil performs corneal transplantation usually using general anesthesia in the operating room. He uses the femto-laser or a circular blade (trephine) to remove a full- thickness disc of most of the cornea and fixes a new disc of clear human donor cornea in its place, using ultra-fine, ultra-strong sutures. Long and ongoing experience in corneal transplantation One of the best known eye surgeons in Egypt and the Middle East, Dr Khalil Master Thesis at Cairo University back in 1989 was on an advanced technique on keratoplasty, the triple procedure (keratoplasty with cataract surgery and intraocular lens implantation). Since then, he has been performing keratoplasty or corneal transplantation surgery and following its tremendous developments and to date. The corneal tissue used in keratoplasty is obtained through eye banks. Dr Khalil Eye Clinic in Cairo gets donor corneas only from reputable US Eye Banks, with full quality certification, and meticulus details of the donor condition. Corneas come from recently deceased donors who have no history of known diseases or other factors that would affect the chance of success of the donated tissue or health of the recipient. • LASIK Vision Correction in Cairo How do we see determines how we perceive the world around us: LASIK vision correction is a laser eye surgery intended to reduce a person’s dependency on his glasses or contact lenses. LASIK is currently the most widely performed elective surgery in the world. LASIK became more and more safer with the introduction of the new femto-laser technology or what is called “ultralasik, intralasik, femtolasik, iLASIK”. Dr Khalil Eye Clinic in Cairo, Egypt offer all these modern LASIK and Femto-LASIK treatment options conveniently. LASIK vision correction utilizes the excimer laser to treat nearsightedness, farsightedness, and astigmatism. LASIK procedure permanently remolds the shape of the cornea, the clear covering of the front of the eye to help you see more clearly. Astronauts and professional divers undergo LASIK: When done properly, LASIK is a safe procedure approved by NASA for its astronauts. Together with other laser vision correction, LASIK has been chosen by millions in Egypt and around the world to correct their eyesight errors such as nearsightedness, farsightedness and astigmatism, to get the best un-hindered vision quality. Some people and some professions are particularly needy to see the best possible eyesight without the need or intervention of glasses or contact lenses. One example are sports-people. In most cases, LASIK is pain-free operation, and takes about 8-10 minutes for both eyes. Best experience, best equipment, for best results: Ongoing investment in the most advanced and best technology combined with a commitment to the highest levels of patient care puts Dr Khalil Eye practice in Cairo, Egypt, in the forefront of best laser correction clinics, centers and hospitals managing these eyesight errors. LASIK outcomes can be enhanced by iDesign, a technology that personalizes your treatment to achieve the very best results. The recovery time for LASIK patients is speedy too, allowing you to start living life to the full almost straight away. The success rates are astoundingly high. Patients are encouraged to have a detailed consultation with Dr Khalil prior to the day of surgery. For the best results of LASIK, at Dr Khalil Eye Clinic you will be well tested to ensure suitability. • MODERN CATARACT SURGERY If you have an eye cataract.. If your vision has faded as a result of cataracts, your likelihood of recovering more youthful vision with modern cataract surgery is exceedingly high, and you will likely enjoy a rapid recovery of enhanced vision with little disruption to your lifestyle after cataract surgery. Modern advances in cataract surgery and its skills have significantly bettered its outcomes. The recent addition and success of various intraocular lens options that can be implanted during modern cataract surgery like: multifocal, trifocal intraocular lenses, and toric intraocular lenses has helped in the simultaneous correction of presbyopia, myopia, hyperopia and astigmatism. These developments in cataract surgery have proved to be a huge leap forward in the outcome following cataract surgery. They offer a much better quality eyesight. The lenses can free the patient from the need to even reading glasses addition after surgery, and justifiably enough have added the name REFRACTIVE to modern cataract surgery. You will have a full consultation with Dr Khalil before your cataract surgery, following which, your best options will be decided. After undergoing full consultation with in-office testing by Dr Ahmed Khalil and some other measurements, cataract operation is usually scheduled in an outpatient surgery center. Anesthesia is provided by a highly specialized anesthesiologist, and medical clearance is required from your internist. Most patients can expect to spend several hours at the surgery center, however the procedure itself usually takes less than 10 minutes. - The best experience in cataract surgery Among the leading best cataract surgeons in Cairo, Egypt, Dr Khalil is accessing all the cutting edge developments in modern cataract surgery, coupled with continuous knowledge updates in most international meetings concerned with cataract surgery around the globe (here shown during a Live Surgery transmission during a World Ophthalmology Congress). • COMPREHENSIVE EYE CONSULTATION AT DR KHALIL EYE CLINIC It is important to check your eyes regularly. Whether or not you need glasses, the health of the eyes should be checked every two years. If you have a family history of eye problems or wear contact lenses more frequent testing may be required. Dr Ahmad Khalil will carry out a full comprehensive eye evaluation during your visit to his eye clinic. As well as eye disease, Dr Khalil will also check for any problems caused in the eye by other general health issues such as diabetes and high blood pressure. Full Eye Consultation with Dr Khalil usually takes little more than half an hour and routinely check for the following problems: - Refractive error - Anterior eye conditions - the presence and if present, the severity of Cataracts - Eye Pressure to exclude or evaluate Glaucoma - Age related macular degeneration If any serious eye disease is detected during the course of testing Dr Khalil will ask for further investigation and then recommend any necessary treatment. Dr Khalil will fully explain any problems if detected and is available to answer any questions you might think of at the time, or later. • SQUINT TREATMENT AND SURGERY Treatment of squint Strabismus is the deviation of the axis of vision for one eye from the other, meaning that the direction of the right eye differs from the left eye. In most cases, the squint appears in childhood with an incidence ranging between 2-4%, and in a few cases the squint may appear in adults, and the strabismus may be present permanently or intermittently, i.e. it appears sometimes and disappears at other times, and in cases of strabismus, the eye may It deviates in any direction: inward (about forget), outward (around a monster), or up or down. And Dr. Ahmed Khalil examines the child with squint early, and in many cases the appropriate treatment is the use of glasses and a cover for one eye, and this is completely sufficient for the complete treatment of squint in the child without the need for surgery, and on the other hand, if the squint is not treated early in children, the child It is prone to the occurrence of the so-called pixel of the eye, as it is not possible to fix the strength of vision in the lazy eye after that in any way, and for adults, the strabismus operation is the goal of which is to correct the position of the eyes so that there is no turning around, without this leading to a correction of vision in the weak eye. The causes of strabismus vary, including the following: - Weakness in the muscles that move the eye or weakness in the nerve signal Link to the muscles of the eye - move the eye or weakness in the nerve signal Link to the muscles of the eye - muscles that move the eye or weakness in the nerve signal Link to the - Weakness in the muscles that move the eye or weakness in the nerve signal Link to the muscles of the eye STRABISMUS TREATMENT In cases around children, the goal of the treatment is to preserve vision, repair the position of the eyes, and restore the stereoscopic image by both eyes, and the treatment of strabismus depends first on the cause of the strabismus in the child, and because the vast majority of cases around children are due to poor eyesight. … and with the start of using the glasses, Dr. Khalil will ask the parents for a cover for one of the eyes to properly exercise the eyes. • ICL Implantation The ICL The ICL is a very fine and delicate lens, which is implanted inside the eye over the normal lens to correct vision errors. Dr Khalil use the ICL to correct short sightedness (myopia), long-sightedness (hyperopia), and astigmatism. Naturally, he performs this after fully judging suitability. The ICL is the best choice he offers to his patients who require ultra- precision in their visual demands. That’s because it gives excellent vision quality when done properly. What is the ICL? The ICL (implanted contact lens, implantable collamer lens) is a synthetic lens to be implanted over the normal human lens (phakic intraocular lens). It is inserted between the iris and the eye’s natural lens. The ICL is capable of correcting a wide range of myopia, hyperopia and astigmatism without the removal of corneal tissue. The ICL is invisible & undetectable! As the ICL is tiny and soft, it is folded up and injected inside the eye through in seconds through a tiny self sealing opening. Once injected, the ICL unfolds into position in the liquid between the iris and natural lens. ICL corrects the eyesight usually without any further treatment being necessary (the ICL can also be removed or replaced, if required, in another simple procedure). THE ICL IS IMPLANTED THROUGH A VERY SMALL OPENING IN THE EYE, AND STABILISED IN ITS PLACE WITHIN FEW MINUTES The ICL provides very predictable refractive outcomes and best quality of vision due to its placement inside the eye, as well as its optical performance. The lens is made of a superior lens material called Collamer® which provides unparalleled biocompatibility and is accepted by the eyes as not being a ‘foreign tissue’. This means that the body does not react or reject the ICL as a foreign body. Other special features of Collamer are its optical clarity and the flexibility and elasticity that allow it to be folded up and then to recover its correct shape in the liquid inside the eye. This lens has been successfully used since 1993, and still have undergone significant improvements since then. • INTRAVITREAL INJECTIONS An intravitreal injection is a process that delivers a specific drug directly into the eye near the retina Intravitreal injections are done in the jelly-like substance that fills the larger part of the eye which is the vitreous cavity. Delivering drugs in this space is the best method to send the required treatment to the need part of the retina or other parts in the back of the eye. Intravitreal injections are used to treat some retinal diseases that were previously intractable to treatment methods such as cases with severe Diabetic retinopathy and center of vision (macula) hemorrhage associated with aging in some people (age related macular degeneration, ARMD), as well as some cases of thrombosis of the retinal vessels. Each case has its own indications and methods of injection, and Dr Ahmad Khalil will tailor the injection dose and technique to your precise condition To have the highest possible standards of safety and excellence, Dr Khalil performs his intravitreal injections inside the opthalmic eye theater, with the highest sterilization environment, similar to what is followed in any sophisticated operation inside the eye. • TREATMENT OF CONGENITAL GLAUCOMA Treatment of congenital glaucoma is essentially surgical: Treatment of glaucoma in children is essentially surgical, with special surgeries designed and dedicated to the infant eye. For the treatment of congenital glaucoma, glaucoma eye drops cannot be used for any long time. Glaucoma eye drops have a weak efficacy in the treatment of congenital glaucoma in children. In addition, potential major side effects on the general health can be caused by the use of eye drops in the treatment of congenital glaucoma. Finally, there is no sufficient long-term studies on their role in scientific literature. SURGERY FOR CONGENITAL GLAUCOMA Conventional techniques of glaucoma surgery used in adults have a very low success rate when used in children. The commonly used chemicals which augment the success of adult glaucoma surgeries are effective only for a short term in glaucoma in children. These surgeries can lead to significant side effects later on in life. Dr Khalil is pioneering his own modifications of the best successful technique of a children glaucoma surgery called trabeculotomy. He has been invited to present his technique of modified trabeculotomy and his management of infants with congenital glaucoma in tens of prestigious international conferences. Along the past years, he has treated over 1000 children successfully with his technique. He is following up most of these children as they grow up with healthy seeing eyes.
