Keratoconus literally means ‘conical or cone-shaped cornea’. The cornea is the clear tissue that covers the front of the eye, and is its major focussing surface. It is normally round but with keratoconus, it becomes stretched and thin near its centre, and slowly transforms to a cone shape. The distorted cornea affects the focusing ability of the eye impairing the vision, due to associated irregular astigmatism and myopia. Keratoconus patients develop progressive corneal bulging/protrusion due to an weakness of the corneal collagen. Keratoconus usually begins in the late teens to early 30s and affects men and women. It is significant in 1:2000 of people, but mild cases occur up to ten times more often. One or both eyes may be affected. It may begin in one eye and later involve the other. KERATOCONUS SYMPTOMS The main symptom is blurred vision The main symptom is blurred vision even with glasses. Patients usually seek corrective lenses at first for driving or reading. Keratoconus at this stage is difficult to differentiate from other more common vision defects. The glasses prescription increases with time and usually shows high levels of myopia (short sightedness) and astigmatism. (distortion of vision) Some patients have glare driving at night or from sunshine. If the distortion of the cornea worsens, so will vision. This happens in some patients but in many the condition stabilises. It is not possible to predict whether keratoconus will be progressive or at what rate it will worsen but in general the younger the age and the more severe the disorder when it begins, the more rapidly it is likely to deteriorate. KERATOCONUS TREATMENT IN CAIRO - COLLAGEN CROSS LINKING A transformation in keratoconus treatment that stops it from getting worse Collagen cross linking is an important new treatment that stops keratoconus from getting worse. Cross linking increases the strength of the collagen in keratoconic corneas by 3–4 times. In keratoconus it is most effective after collagen cross linking, laser or rings implantation have been done to stabilise the cornea and normalise its shape as much as possible - RGP CONTACT LENSES, ROSE-K LENSES Contact lenses do not change the keratoconus, but cover the protrusion on the cornea so neutralising it. Normal vision is restored to all except advanced cases. Lifelong wear is necessary. - RINGS IMPLANTATION INSIDE THE CORNEA It is to reduce excessive curvature in advanced keratoconus. These are small pieces of plastic inserted into a channel created in the cornea to stretch it and bring it back to near its original configuration. They can’t be felt and require no maintenance. They are normally invisible except on close inspection when they look like a contact lens on the eye. - CORNEAL TRANSPLANTATION A long-established treatment for the most severe cases The center of the patient’s own cornea is completely removed and replaced by another corneal transplant. The transplant is sutured in place of the conical cornea. This is the longest-established treatment for severe keratoconus. - LASER Laser treatment can be used to treat keratoconus in some rare occcasions by using a cornea topographer to guide the laser, or what is called topography-guided treatment. This is an important development, but Dr Ahmad Khalil reserves it to very rare cases, where the cornea is stable, with a minimal refractive error untreatable by other means. • GLAUCOMA TREATMENT​ What is Glaucoma? Glaucoma is one of the leading causes of blindness in the world. It is a disease that typically affects older people, but it can occur at any age, even in the newborn. Loss of vision is preventable if the disease is detected early and treatment is started. Glaucoma is typically known as a disease of high pressure inside the eye causing damage to its structures, but some eyes are sensitive even to lower pressures-normal tension glaucoma. Types of Glaucoma THERE ARE TWO MAIN SUB-GROUPS OF PRIMARY GLAUCOMA: 1- Open Angle Glaucoma Despite the angle of the eye being wide and open, the tiny microscopic drainage channels outside the angle are blocked. This leads to what we call open angle glaucoma, which is a slowly progressive condition, and because it is a slow process, it can often be missed, This is what we call “the silent thief of sight” 2 - Angle Closure Glaucoma (also known as closed angle glaucoma) occurs more rapidly when the flow of fluid inside the eye cannot pass through the narrow angle in the front of the eye, where drainage usually occurs, causing a rapid rise in pressure inside the eye. THERE ARE OTHER 2 ENTITIES WITH SOME RESEMBLANCE TO GLAUCOMA Ocular hypertension occurs when the pressure of the eye is repeatedly measured as being high, and yet the eye displays no signs of glaucoma damage. Normal tension glaucoma (normal pressure glaucoma) is the opposite, where the nerve at the back of the eye shows signs of glaucoma damage and yet the pressure is always recorded as being within normal limits. THERE ARE SOME EYE DISEASES WHICH CAN RESULT IN SECONDARY GLAUCOMA Among these are eye inflammation, the growth of new vessels inside the eye, which may occur in connection with diabetes or after vascular occlusions at the back of the eye. CONGENITAL GLAUCOMA In some rather rare instances, glaucoma can occur in young infants. This is called congenital glaucoma and can be inherited. This type of glaucoma can be devastating if not diagnosed and operated early enough. Glaucoma surgery, if thought useful will reduce to a large extent the need for closer follow up visits. • LASER TREATMENT OF THE RETINA RETINAL LASER The laser is an instrument that produces a pure, high-intensity beam of light energy. The laser light can be precisely focused onto the retina (the light sensing tissue at the back of the eye), selectively treating the desired area while leaving the surrounding tissues untouched. The absorbed energy creates a microscopic spot to destroy lesions or weld tissues together. Retinal laser treatment is a painless outpatient procedure, lasting about 10-30 minutes. It may require more than one session to complete the treatment. In most of the cases, the aim of laser treatment is to try to prevent further loss of vision. It cannot regain the vision if already lost due to retinal disease. After the laser treatment, the patient may require to undergo repeat Fluorescein Angiography at a later date to monitor the response, and may require more laser if indicated. DIABETIC RETINOPATHY Diabetes milletus can damage small vessels of the retina, causing them to leak fluid and blood. These changes decrease vision if the central part is affected. Laser is used to seal these leaks. However laser is done to prevent or retard further loss of vision and not to improve vision. In severe changes when new blood vessels have grown (Proliferative Diabetic Retinopathy), multiple sessions of laser are needed to produce the desired effect. This is called Pan Retinal Photocoagulation and is highly effective in preventing severe visual loss. RETINAL TEARS AND HOLES In a retinal tear or a hole without a retinal detachment, laser is done prophylactically to seal the hole and prevent or limit the detachment. AGE RELATED MACULAR DEGENERATION In patients with wet type of AMD not involving the central part, laser can be done to close the Choroidal Neovascular Membrane. This can prevent severe visual loss. However, in patients with central involvement, other specific techniques involving injections of special materials in the eye to prevent further proliferation, and stop existing ones. RETINAL VEIN OCCLUSIONS In venous occlusions, the central retina may swell up due to fluid collection leading to reduced vision. Such cases require laser to decrease the swelling, which may help in improving the vision. Venous occlusions may also lead to new vessel formation that might cause glaucoma (9eye pressure rise), laser causes these vessels to regress. • Keratoplasty Keratoplasty, Corneal Grafting Corneal transplantation or keratoplasty is a surgery to replace an opaque or diseased cornea with a best clarity corneal tissue, which is obtained from a donor. In most cases, the squint appears in childhood with an incidence ranging between 2-4%, and in a few cases the squint may appear in adults, and the strabismus may be present permanently or intermittently, i.e. it appears sometimes and disappears at other times, and in cases of strabismus, the eye may It deviates in any direction: inward (about forget), outward (around a monster), or up or down. The ICL (implanted contact lens, implantable collamer lens) is a synthetic lens to be implanted over the normal human lens (phakic intraocular lens). The ICL is invisible & undetectable! As the ICL is tiny and soft, it is folded up and injected inside the eye through in seconds through a tiny self sealing opening. ICL corrects the eyesight usually without any further treatment being necessary (the ICL can also be removed or replaced, if required, in another simple procedure). THE ICL IS IMPLANTED THROUGH A VERY SMALL OPENING IN THE EYE, AND STABILISED IN ITS PLACE WITHIN FEW MINUTES The ICL provides very predictable refractive outcomes and best quality of vision due to its placement inside the eye, as well as its optical performance. For the treatment of congenital glaucoma, glaucoma eye drops cannot be used for any long time. Glaucoma eye drops have a weak efficacy in the treatment of congenital glaucoma in children. In addition, potential major side effects on the general health can be caused by the use of eye drops in the treatment of congenital glaucoma. Finally, there is no sufficient long-term studies on their role in scientific literature. SURGERY FOR CONGENITAL GLAUCOMA Conventional techniques of glaucoma surgery used in adults have a very low success rate when used in children. The commonly used chemicals which augment the success of adult glaucoma surgeries are effective only for a short term in glaucoma in children. These surgeries can lead to significant side effects later on in life.
